REPORTS
- A Final Report must be sent to the Foundation by the date specified in the grant agreement. This Report may be sent earlier than the date specified if all the funding has been utilized.
- Please title the Report by the year of the grant check date, not the organization’s fiscal year or grant expenditure dates.
- The Report (Final or Interim) for a previous grant must be received before a new grant will be approved.If you have received an invitation to submit a new Proposal and are not yet ready to submit the Final Report showing full expenditure of the previous grant, please create an “Interim Report” by following the guidelines below to provide as much information as possible. Submit this document with the new Proposal and follow up with the updated Final Report at the specified due date when the prior year’s funding has been fully disbursed.
- The Report (no more than three or four pages in total) should respond to the following questions. It is most helpful to us when you use a simple enumerated list of a) through f) with brief answers to each of the questions.
- How was the Wallace Genetic Foundation funding used?
- What was accomplished?
- A frank assessment of whether the intended outcomes were realized.
- A description of the challenges faced and what was (or will be) done to address them.
- Given the outcomes, what was learned?
- How will any benefits be sustained over time?
- Note additional funding that was secured for the project.
- Budget
- If the grant was for General Support, compare the organization’s actual revenues and expenses with the anticipated budget that was previously submitted, noting any relevant variances with a brief explanation.
- If the grant was for a Specific Project, compare the Project’s actual line-item budget to that of the proposed budget, with a brief explanation of any variances between the two. (The most helpful way to do this is often just to add a new column o the proposal budget previously submitted to show the Actual Expenses.)
Submission:
The Report should be submitted as follows:
- in electronic form to wgfdn@wallacegenetic.org, AND
- in electronic form to the primary board member with whom the organization works, AND
- in hard copy by regular postal mail to the Foundation office in Washington DC (4910 Massachusetts Avenue, NW, Suite 221, Washington DC, 20016).
Do not use other delivery services, such as FedEx or UPS, or require a signature or special delivery as there is seldom someone in the office at the usual times of delivery. We have found the U.S. Postal Service Priority Mail service (again, without signature or 2-day special delivery required) to be reliable if you want to include tracking.
